Do you need a night out for Valentine's Day? Mack Kidz Stay-N-Play offers a fun and safe environment for kids 3 and older! 

The program is from 6:30 pm - 11pm. Either February 14th or February 15th, 2020.

• If you do not want your child to participate in the dinner that is included, please provide a sack lunch or snacks. Please label anything brought with your child’s full name and preparation directions. Our facility is capable of refrigeration, preparation or heating. • All children much be fully potty trained. Ages 3+ years.

• You are more than welcome to bring in a pillow or sleeping bag to this event. Please not that we are not responsible for any lost or stolen items.

• There are no refunds or transfers.

• Please do not bring your child if they are sick. We will not allow children that are sick to be dropped off; any child that becomes ill during the program will need be to be picked up as well.

• All children will need to be signed in and out by an approved parent/guardian. The sign in/out sheet will be in the lobby. A photo ID will be required at pick-up.

• Late Pick Up: $12 for every additional 30 minutes.

I understand that the risk of injury is inherent in any activity physical in nature. I, hereby consent to my child’s presence at Mack Kidz Play-N-Stay. I do, hereby, for myself, my child, heirs and executors forever waive and release Mack Kidz Play-N-Stay, its employees and agents from any and all claims, actions, demands, rights and damages of any nature whatsoever, that I or my child may have at any time against Mack Kidz Play-N-Stay, its directors, officers, employees and agents. It is hereby understood that Mack Kidz Play-N-Stay is not responsible in any way for damage of loss of any personal property which the child or parent/guardian brings onto the premises of Mack Kidz Play-N-Stay.

AUTHORIZATION FOR MEDICAL TREATEMENT OF A MINOR I, the undersigned, parent/guardian of the above names person, a minor, do hereby authorize Mack Kidz Play-N-Stay as agent(s) for the undersigned to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is to be rendered under the general or special supervision of any physician and surgeon licensed under the provisions of the Medical practice Act on the medical staff of any accredited hospital, where such diagnosis or treatment is rendered at the office of said physician or at said hospital. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care which the aforementioned physician in the exercise of his/her best judgment may deem advisable. I further agree to be responsible for all costs associated with any treatment provided in compliance or attempted compliance in good faith with this consent.
